Pfizer’s Drug-Testing Strategy in Nigeria

The drug development process is long, risky, and expensive. It can take 10 years and cost in excess of $500 mil- lion to develop a new drug. Moreover, between 80 and 90 percent of drug candidates fail in clinical trials. Pharmaceutical companies rely upon a handful of successes to pay for their failures. Among the most successful of the world’s pharmaceutical companies is New York- based Pfizer. Given the risks and costs of developing a new drug, pharmaceutical companies will jump at opportunities to reduce them, and Pfizer thought it saw one.

Pfizer had been developing a novel antibiotic, Trovan, that was proving to be useful in treating a wide range of bacterial infections. Wall Street analysts were predicting that Trovan could be a blockbuster, one of a handful of drugs capable of generating sales of more than $1 billion a year. In 1996, Pfizer was pushing to submit data on Trovan’s efficacy to the Food and Drug Administration (FDA) for review. A favorable review would allow Pfizer to sell the drug in the United States, the world’s largest market. Pfizer wanted the drug to be approved for both adults and children, but it was having trouble finding sufficient numbers of sick children in the United States to test the drug on. Then a researcher at Pfizer read about an emerging epidemic of bacterial meningitis in Kano, Nigeria. This seemed like a quick way to test the drug on a large number of sick children.

Within weeks a team of six doctors had flown to Kano and were administering the drug, in oral form, to children with meningitis. Desperate for help, Nigerian authorities gave the go-ahead for Pfizer to give the drug to children (the epidemic would ultimately kill nearly 16,000 people). Over the next few weeks, Pfizer treated 198 children. The protocol called for half the patients to get Trovan and half to get a comparison antibiotic already approved for the treatment of children. After a few weeks, the Pfizer team left, the experiments complete. Trovan seemed to be about as effective and safe as the already approved antibiotic. The data from the trial were put into a package with data from other trials of Trovan and delivered to the FDA.

Questions were soon raised about the nature of Pfizer’s experiment. Allegations charged that the Pfizer team kept children on Trovan, even after they failed to show a response to the drug, instead of switching them quickly to another drug. The result, according to critics, was that some children died who might have been saved had they been taken off Trovan sooner. Questions were also raised about the safety of the oral formulation of Trovan, which some doctors feared might lead to arthritis in children. Fifteen children who took Trovan showed signs of joint pain during the experiment, three times the rate of children taking the other antibiotic. Then there were questions about consent. The FDA requires that patient (or parent) consent be given before patients are enrolled in clinical trials, no matter where in the world the trials are conducted. Critics argue that in the rush to get the trial established in Nigeria, Pfizer did not follow proper procedures, and that many parents of the infected children did not know their children were participating in a trial for an experimental drug. Many of the parents were illiterate, could not read the consent forms, and had to rely upon the questionable translation of the Nigerian nursing staff. Pfizer rejected these charges and contends that it did nothing wrong.

The FDA approved Trovan for use in adults in 1997, but it has not approved the drug for use in children. Trovan was launched in 1998, and by 1999 there were reports that up to 140 patients in Europe had suffered liver dam- age after taking Trovan. The FDA subsequently restricted the use of Trovan to those cases where the benefits of treatment outweighed the risk of liver damage. European regulators banned sales of the drug

Riding the Emotional Roller Coaster

 

Louise Damiani’s work is an emotional roller coaster most days. The oncology nurse at CentraState Healthcare System in Freehold Township, New Jersey, soars with joy when patients beat their cancer into remission. Then there are the low points when her patients are given grim news about their cancer. She also battles with the frustration of office politics.

But even after a long shift, Damiani doesn’t let her negative emotions surface until she gets into her car and heads home. “You have to learn how to pick and choose and not bring that emotion up,” Damiani advises. “You say, ‘OK, I can deal with this. I can focus on the priority, and the priority is the patient.’”

As well as managing her own emotions, Damiani has mastered the skill of creating positive emotions in others. She recently received an award in recognition of her extraordinary sensitivity to patients’ needs and concerns. For example, one of Damiani’s patients wanted to return to her native Mexico but, with an advanced stage of cancer, such a journey wasn’t possible. Instead, Damiani brought “Mexico” to the hospital by transforming a visitors’ lounge into a fiesta-type setting and inviting the patient’s family, friends, and hospital staff to attend the special event.

Lisa Salvatore, a charge nurse at the recently built Leon S. Peters Burn Center in Fresno, California, also recognizes that her job involves supporting patients’ emotional needs, not just their physical problems. “With burns, you don’t just treat something on the outside,” she says. “You treat something on the inside that you can’t see.” Salvatore also experiences the full range of emotions, including the urgency of getting burn patients out of emergency within an hour to improve their prospects of recovery. “I like high stress. I like trauma,” she says. Still, she acknowledges the emotional challenges of treating children with burns. “I deal with it and then I cry all the way home. I just sob on my way driving home.”

Anil Shandil, a medic from the 328th Combat Support Hospital in Fort Douglas, Utah, has witnessed more severe burns and injuries than most medical professionals. For two years at the Landstuhl Army Regional Medical Center in Germany, he aided soldiers who had been wounded in Iraq or Afghanistan. The tour of duty was extremely emotionally taxing. “You get a lot of severed limbs, a lot of traumatic brain injuries, a lot death and dying,” says Shandil. “So the compassion fatigue is rather high.” People who work closely with victims of trauma often suffer compassion fatigue, also known as secondary traumatic stress disorder. The main symptom is a decreasing ability to feel compassion for others.

In spite of the risk of compassion fatigue, Shandil has volunteered for an even more challenging assignment: He and 85 other soldiers in the 328th are now in Iraq providing medical care for Iraqi detainees being held there by the U.S. military. So, along with managing emotions from constant exposure to trauma cases, these medics must also show respectful compassion to those who fought against their American comrades. Shandil knows it will be hard. “Yes, these are people who were not kind to us. But as a medic, it’s our job to care for them, no matter if that is your friend or your enemy.”

Child Molestation Case

You and your partner are detectives working in plain clothes. For several days, you have been staking out an elementary school playground where an adult male reportedly has been approaching the children. One particular morning, you have observed a man in his car parked alongside the school yard. When he calls a little girl over to his car and she approaches him on the driver’s side, you exit from your unmarked vehicle and approach his vehicle from the passenger side so that he cannot see you easily in his rear view mirror. When you get just behind his car on the passenger side, you observe that he has his pants open. With his left hand, he is stroking the girl’s hair.

You call for your partner over the radio and move to arrest the suspect. Showing him your badge, you order him to close his pants and get out of his car. You handcuff him and place him in the back of your unmarked vehicle, while your partner returns the girl to the safekeeping of school administrators.

After telling the suspect of his right to remain silent, you begin interrogating him, recording his name, address, and so forth while awaiting your partner’s return.

When your partner comes back, he gets in the rear of your vehicle with the suspect (a violation of department regulations, unless there is reason to believe that the passenger is suicidal). You remind your partner that riding in the back is a violation of the rules, but he stays there anyway; tells you to drive to the station; and turning to the suspect, calls him a “maggot.” As you drive, your partner starts punching the suspect repeatedly in the face and the crotch. He even leans back and, bracing himself against the car door, kicks the suspect in the chest and shoulders with the heel of his shoe. When the suspect turns to avoid the blows, your partner kicks him in the back and kidney areas. Throughout this beating, the suspect offers no resistance of any kind. You hear the sounds of the beating and, looking in the rear view mirror, you see your partner striking the suspect. After about a minute, you pull the vehicle over, open the rear door and persuade your partner to get out of the back seat and come sit beside you in the front.

After the suspect is booked and in a holding cell, your partner tells you he will write a report that includes the assertion that the child molester forcefully resisted arrest and sustained injuries during the scuffle to handcuff him. He tells you that he will write the report and sign it, and that you do not have to be associated with its contents.

At his trial, the offender pleads guilty to child molestation charges and is sentenced to 3 years in prison, to be served at a treatment center for sexual offenders. Soon afterwards, your partner is sued by the offender for physical abuse and injury. Claiming kidney damage, loss of two teeth, civil rights violations, and psychological trauma, the convicted molester is asking a civil (not a criminal) court for an award of $2 million from your department and $500,000 from your partner. You receive a subpoena from the court to appear at the trial. In addition, the Internal Affairs Division will launch its own investigation of the allegations, and it informs you that you will be called in to make a statement under oath at a hearing about the events. If the department finds sufficient grounds to believe that your partner acted brutally, he will be suspended, or more likely, fired; if the civil jury finds that he was abusive, he may be forced to sell his home and car to pay the award to the victim. You may also be disciplined for failure to protect your prisoner. Comments in the locker room by other officers make it clear that, given the nature of the victim, they expect you to do whatever is necessary to protect your partner from being held accountable, as well as to protect yourself.

There are several ways to analyze a work of literature. Two of the most common are below:

1. A formal approach–which examines one (or more) of the literary elements in a work and explains how it relates to the whole, thus contributing to a text’s overall meaning or significance. Possible elements to consider include plot, character, theme, setting, conflict, structure, point of view, genre, style, tone, imagery, symbolism, irony, narrator, foreshadowing, paradox, allusion, metaphor, rhythm, rhyme, etc.

2. A critical approach–which examines a work by relating it to the historical, social, cultural, or political situations in which it was written to show how the author was influenced by personal experiences, events, prevailing attitudes, or contemporary values.

Each of these approaches requires careful reading of the primary text, as well as a debatable claim that will need to be supported or “proved” with evidence (direct references, quotations) from the text. Remember, writing about literature is a way to broaden your own understanding and to share your own reading experience. While the second option (a critical approach) will obviously involve some background reading, you should not be going to outside sources with the idea of creating a “cut and pasted” paper of others’ ideas. Your primary concern for either approach should be an examination of the work of literature you selected.

Example of a poor thesis: “Trifles is a play about a woman who murders her husband.” (not debatable)

Example of a good thesis with a formal approach: “Mrs. Wright’s bird is the symbolic key to “unlock” the reason why she killed her husband. (debatable! Requires textual support to “prove.”)

Example of a good thesis with a critical approach: “Mrs. Peters and Mrs. Hale share experiences from their own lives as country wives that make clear why they are empathetic to Mrs. Wright’s situation and right to withhold their discovery from their investigators.” (debatable! Requires feminist analysis of story to “prove.”)

Because you are essentially arguing that your perspective is a valid one, you have to support it effectively with evidence from the primary source that you select (direct references to specific quotations, lines, passages, scenes, etc.). Remember that an analysis is not driven by plot. It’s driven by the “idea” that you want to communicate about the story or poem. If you consult secondary sources, be sure that quotations from them do not dominate your essay. You are not writing an informational report but rather a literary analysis that requires a close reading of the text.

ESSAY INSTRUCTIONS:

Your essays should be in MLA Style and approximately 3-4 pages (not including the Works Cited page). In the upper left-hand corner of the paper, place your name, the professor’s name, the course name, and the due date for the assignment on consecutive lines. For this Application, which you will begin this week and complete by the end of next week, you are asked to analyze a core policy about health or healthcare. Examples might include policies on scope of practice for healthcare practitioners or on health insurance for children. After selecting your policy, locate resources to help you understand the specifics of it, including how its programs are organized and financed, how the services are delivered, and, generally, how well the policy has been implemented to date.

Make sure to select a policy on which you can focus in some detail.
